COMMUNITY ALTERNATIVES FOR YOUTHS AND FAMILIES IN CRISIS

Community Alternative services are provided in direct collaboration with the Indiana Department of Mental Health and other local partner agencies.  Our Community Alternatives (CAPRTF)  program offers support to youth ages 6-21 who present with a high level of need for intensive services due to behavioral and/or mental health issues. Using the Wraparound Systems of Care model, a Child and Family Team is first identified. We then collectively assist the family in identifying and building their strengths and skills so that the youth can thrive in their home and community. Connections offers the following CAPRTF services across the State of Indiana:

Clinical Consultation:   Our clinical consultation services provide direct professional support to caregivers of youth with intensive behavioral issues in order to improve the participant’s independence and inclusion in their community. This service includes assessment, development of a home treatment/support plan, training and technical assistance to carry out the plan, monitoring of the participant and other providers in the implementation of the plan, and participation in Child and Family Team meetings. Crisis counseling and family counseling may be also be provided.

Habilitation:   Habilitation services for children and adolescents focus on enhancing the participant’s life skills and social functioning. This is accomplished through developing skills in identification of feelings; anger and emotional management; how to give and receive feedback, criticism and praise; problem-solving; decision making; controlling assertive behavior; learning to resist negative peer pressure and develop pro-social peer interactions; improve communication skills with peers and adults; optimize developmental potential; address substance use and abuse issues; build and promote positive coping skills; and encouraging positive community connections for the development of natural supports for the child and family.

Wraparound Technician: Wraparound Technicians assist the family and child with gaining access to services and ensure that families are aware of available community-based services and other resources, such as educational and public assistance programs. They also monitor use of services and engage in activities that enhance access to care, improve efficiency and continuity of services, and prevent inappropriate use of services. Wraparound Technicians monitor the health and welfare of the youth and communicate that progress to the Child and Family Team. Crisis intervention services may also be provided as needed.
