SCHOOL BASED TREATMENT SERVICES

Care Facilitation:  Our Care Facilitators assure students’ basic health and safety needs are met through creative case management services designed to actively engage family involvement in the resolution and by linking them to valuable community resources.  Care Facilitators work closely with families to explore and understand the youth’s history, friendships, and any peer and family relationships which may be contributing to challenging behaviors at home and school. They also assure close communication with district staff on student progress and work closely with our physician to assure the students medical needs are met. 

Group Psychotherapy: Our school based group counseling services provide therapeutic support to children who present challenging behaviors in the classroom setting. Led by masters level therapists, our curricula focuses upon teaching students techniques to cope with typical adolescent issues such as bullying, internet safety, dating violence, illegal substance use, aggression & anger  control, gang affiliation, personal hygiene, etc.  

Individual Psychotherapy: Our masters level therapists provide individualized therapeutic support for children whose serious emotional disorders prevent them from functioning in the traditional classroom setting. Our therapists develop treatment plans to identify students behavioral health goals and through their interventions strive to reconcile any difficult family and peer relationships; as well as, decrease the frequency and intensity of any antisocial, self abusive, or disruptive behaviors. All treatment plans are reviewed routinely by our agency HSPP/psychologist as well.

Psychiatric/ Medication Management Services: Connections' Child and Adolescent Psychiatrist is available on site at the school for direct patient consultation, supervision, and medication management services on a weekly basis.  Our MD/ physician is also available 24 hours per day for crisis triage, parent education, and for the support of our agency staff and district teaching staff.  Additionally, in order to assure the emergency psychiatric need of our students are met; we also maintain direct hospital admission privileges within several local hospitals.  

Transition Coordination: Our Transition Coordinators organize and facilitate opportunities for community based volunteer and exploration activities to promote generalized learning, reciprocity, responsibility, and vocational education for our students. They also seek to assure the transition between school programs for enrolled students are planned well in order to  reduce the stress of changes for them. In addition students receive weekly instruction about issues which may impact their ability to be successful in general education settings, such as, bullying, hygiene issues, peer influences, etc.
